Ability To add a section to default templates that outlines treatments and diagnostics that were declined by the owner

Talkatoo, with way too much regularity, cannot record a patient name it does not know. Even stating the patient name and then stating how to correctly spell that name does not prevent the app from making this all to frequent, regular error.
