Thanks for sharing this feedback. This can now be handled with custom instructions for default templates.

You can go to Preferences > Default Templates, select the relevant template, such as SOAP, and add an instruction in the User Instructions field at the top.

For example:
“Add a separate Prognosis portion to the Assessment section when appropriate.”

Or:
“Group related problems from the problem list, list potential differentials for each group, and include an overall prognosis when appropriate. Example: Problem 1, Problem 2: Differential 1, Differential 2, Differential 3. Prognosis: Good.”

This should help address this request and give users more control over their notes without needing a full custom template.
